WHAT YOU NEED TO KNOW
Why I strongly recommend having a valid passport and require a waiver if you choose to travel without one:
• Unexpected emergencies can happen, and without a passport,
returning to the U.S. can become complicated and stressful.
• You may be required to visit a U.S. embassy or consulate to obtain an
emergency passport before you’re allowed to fly home.
• Any additional travel, lodging, or transportation needed while you’re stuck
abroad becomes an out‑of‑pocket expense. Having a passport is a simple step that protects
you from major headaches and unexpected costs.

BIRTH CERTIFICATES
Certified Birth Certificate with Embossed Seal
This can't be a photocopy of your original birth certificate,
or a hospital issued copy
IF YOU WERE BORN IN THE UNITED STATES
Issued by the city, county, or state of birth

Lists applicant’s full name, date of birth, and place of birth

Lists parent(s)’ full names
-
Has the signature of the city, county, or state registrar
-
Has the date filed with registrar's office (must be within one year of birth)

Has the seal or stamp of the city, county, or state which issued it.

Passport Photos
You can have these taken at your local Walgreen's Pharmacy and UPS Store to name a few.
It's taken on a white background, so be sure to wear any other color shirt than white.
